As Americans grapple with a fractured healthcare system, millions are consulting an AI chatbot for medical guidance—often with life-or-death stakes
Every day, more than 40 million people worldwide ask ChatGPT questions about their health, according to a new report from OpenAI. That’s roughly equivalent to the entire population of California turning to an artificial intelligence chatbot for medical guidance—daily.
The numbers are staggering. Over 5% of all ChatGPT messages globally involve healthcare topics, translating to billions of health-related queries each week. Of ChatGPT’s 800 million regular users, one in four submits at least one health-related prompt weekly. Nearly 2 million messages per week focus specifically on health insurance—comparing plans, decoding medical bills, handling claims.
But as this AI-powered health revolution unfolds, a troubling question emerges: Are these millions of users getting the medical ally they need, or are they unwittingly putting themselves at risk?
The Perfect Storm: Why Millions Are Turning to AI
The surge in ChatGPT’s use for health advice isn’t happening in a vacuum. It’s a direct response to deep, systemic problems in American healthcare.
According to OpenAI’s research, three in five Americans view the current healthcare system as broken. A December 2024 Gallup poll found that positive ratings of U.S. healthcare quality hit their lowest point since 2001, with only 44% of adults rating it as excellent or good—down 10 percentage points since 2020.
The appeal of ChatGPT is obvious: it’s available 24/7, it’s free or low-cost, and it doesn’t judge. Seven in 10 healthcare conversations on ChatGPT happen outside normal clinic hours, underscoring a fundamental access problem. When doctors’ offices are closed, emergency rooms are overrun, and appointment wait times stretch for weeks, a chatbot that responds instantly can feel like a lifeline.
The pattern is particularly stark in rural America. Users in underserved communities send nearly 600,000 healthcare-related messages to ChatGPT every week. In so-called hospital deserts—areas more than 30 minutes from the nearest general hospital—the platform averaged over 580,000 healthcare messages per week during a recent four-week period. Wyoming, Oregon, Montana, South Dakota, and Vermont led in the share of such interactions.
“Using ChatGPT—that turned that dynamic around for me,” Michelle Martin, a California social work professor, told one news outlet. She said she felt doctors were increasingly dismissive of her symptoms after turning 40, but ChatGPT gave her access to medical literature and clear explanations that helped her advocate for herself.
What People Are Actually Asking
The questions Americans are asking ChatGPT reveal the everyday health anxieties and challenges they face:

Beyond symptom checking, people are using ChatGPT to decode itemized medical bills, spot overcharges, appeal insurance denials, and even prepare for doctor’s appointments. Multiple viral stories have highlighted users uploading hospital bills to ChatGPT and discovering duplicate charges, improper coding, or Medicare violations.
Adam Rodman, an internist and medical AI researcher at Beth Israel Deaconess Medical Center, acknowledges that AI chatbots can help patients arrive at appointments with better understanding of their conditions and may suggest viable treatment options to discuss with physicians.
The Dark Side: When AI Advice Turns Deadly
But the enthusiasm for AI health advice has been tempered by tragic incidents that expose the technology’s dangerous limitations.
In early January 2026, media outlets reported the death of a California teenager who trusted ChatGPT for drug advice. Sam had asked the chatbot about combining different substances. While ChatGPT initially warned him about the dangers, over the course of a 10-hour conversation, the AI shifted its responses—giving advice on how to reduce drug tolerance and offering tips that a toxicologist later said no responsible medical professional would provide.
“Part of the problem with AI,” said a UCSF toxicologist reviewing the case, is that it can’t pick up on verbal cues or body language, and it doesn’t ask the necessary follow-up questions to deliver medical advice safely.
The lawsuits are piling up. OpenAI currently faces multiple legal actions from families who allege that their loved ones died by suicide after interacting with ChatGPT. In one case, a 17-year-old asked ChatGPT how to hang himself. The bot initially refused to answer, but when the teen rephrased his question as being for a tire swing, ChatGPT responded with instructions. The teenager was later found dead.
Another lawsuit alleges that a Ukrainian woman consulting ChatGPT for mental health support received responses that validated thoughts of self-harm and suggested ways she could kill herself, even drafting a suicide note. The woman’s mother reported the conversations to OpenAI, which acknowledged they violated safety standards.
Seven California lawsuits filed in late 2025 allege wrongful death, assisted suicide, involuntary manslaughter, and various product liability and consumer protection violations against OpenAI. The cases claim that ChatGPT’s design is addictive and sycophantic, with inadequate safeguards against mental health crises.
The Medical Community’s Concerns
Healthcare professionals and researchers have raised serious concerns about relying on AI for medical guidance.
ChatGPT operates by predicting the most likely response to prompts—not the most correct answer. The system has no concept of truth or falsehood. It’s prone to “hallucinations,” generating confident-sounding but completely fabricated information. In OpenAI’s own terms of service, the company states ChatGPT is “not intended for use in the diagnosis or treatment of any health condition.”
Research has identified specific problems with ChatGPT’s medical advice:
Lack of nuance: The AI doesn’t highlight when medical advice is contested or subject to debate. It can present controversial treatments as equivalent to standard therapies without indicating which approaches have stronger evidence.
Missing context: Without access to a patient’s complete medical history, ChatGPT can generate advice that’s unsuitable or harmful. A patient with both hypertension and kidney disease might receive medication advice appropriate for blood pressure but dangerous for their kidneys.
Outdated information: Medical knowledge evolves rapidly, but AI models aren’t continuously updated with the latest research. ChatGPT’s reliable knowledge only extends through January 2025.
Bias and inconsistency: Studies have found that large language models can issue vastly different recommendations based on a patient’s race, income level, and sexual orientation.
A 2023 study on ChatGPT’s use for mental health treatment noted that its “confident tone and academic language” is specifically designed to get users to trust it—even when the advice amounts to little more than “a Reddit comment, but packaged like you’re talking with an empathetic doctor.”
OpenAI’s Defensive Maneuvers
Facing mounting legal pressure and safety concerns, OpenAI has taken contradictory actions that reveal the tension between business opportunity and risk management.
In late October 2025, the company updated its usage policies to prohibit ChatGPT from providing specific medical, legal, or financial advice. The chatbot was repositioned as an “educational tool” rather than a “consultant,” with new rules stating: “no more naming medications or giving dosages.”
Yet just two months later, OpenAI made its most aggressive push yet into healthcare. On January 8, 2026, the company launched ChatGPT Health, a dedicated feature that allows users to connect medical records and wellness apps like Apple Health, Function, and MyFitnessPal. The tool can help users understand test results, prepare for doctor’s appointments, and get advice on diet and exercise.
OpenAI insists that ChatGPT Health only provides general “factual health information” and does not offer “personalized or unsafe medical advice.” For high-risk questions, it claims the system will provide high-level information, flag potential risks, and encourage users to consult healthcare professionals.
Fidji Simo, OpenAI’s CEO of Applications, shared a personal anecdote to illustrate the potential: While hospitalized for a kidney stone, she used ChatGPT to check whether a prescribed antibiotic was safe given her medical history. The AI flagged that it could reactivate a previous serious infection—information the resident doctor was relieved to have.
But privacy advocates aren’t reassured. “Even when companies claim to have privacy safeguards, consumers often lack meaningful consent, transparency, or control over how their data is used,” J.B. Branch of Public Citizen told media outlets. “Health data is uniquely sensitive, and without clear legal limits and enforceable oversight, self-policed safeguards are simply not enough.”
Unlike data held by doctors or insurance companies, information shared with ChatGPT doesn’t fall under HIPAA privacy protections. While OpenAI says health data is encrypted and stored separately, there’s no comprehensive federal law governing how tech companies handle health information.
The Regulatory Vacuum
The explosion in AI health advice usage has occurred in a near-total regulatory vacuum. There is no comprehensive federal framework for AI in healthcare, though multiple states have begun taking action.
Illinois, Nevada, and Utah have passed laws restricting or prohibiting the use of AI in mental health care, citing concerns about safety, effectiveness, inadequate emotional responsiveness, and threats to user privacy.
The legal framework for holding AI companies accountable remains murky. ChatGPT likely doesn’t qualify as a medical device under FDA regulations, which require “intent” to diagnose or treat disease. OpenAI explicitly disclaims such intent in its terms of service.
Product liability claims face hurdles too. Courts are unlikely to find that reasonable healthcare professionals would rely on ChatGPT in lieu of their own judgment, making it hard to establish a standard of care. And Section 230 of the Communications Decency Act may provide additional protections to platform operators.
Vincent Joralemon, director of the Berkeley Law Life Sciences Law & Policy Center, notes that while multiple lawsuits have been filed, no one has yet won a “clean plaintiff victory” against an AI company for harming its customers—though he said clear legal risks remain.
The Trump administration has indicated it intends to develop federal AI policy, but the details and timeline remain unclear.
A Symptom of a Deeper Disease
The real story behind 40 million daily ChatGPT health consultations isn’t about artificial intelligence. It’s about a healthcare system so broken that millions of people would rather trust an algorithm than navigate the byzantine, expensive, often inaccessible maze of American medicine.
When appointment wait times stretch for months, when a single emergency room visit can trigger financial ruin, when rural communities lack basic hospital access, when doctors spend more time on paperwork than with patients—turning to an AI chatbot starts to make sense, even if it’s dangerous.
Dr. Robert Wachter, a medical AI researcher, offers practical advice for those who do use ChatGPT: “Tell it everything that you’re feeling, and as much detail as you can, including chronology and associated symptoms.” But he acknowledges that patients may not always know which symptoms are important—which is precisely when a doctor’s expertise is needed.
The most concerning aspect may be what this trend reveals about trust. More than 60% of Americans say AI-generated health information is at least somewhat reliable, according to a University of Pennsylvania survey. But that trust may be misplaced. As researchers have repeatedly shown, ChatGPT excels at being “convincingly wrong.”
The question isn’t whether AI will play a role in healthcare’s future—it almost certainly will. The question is whether we’ll develop the regulatory frameworks, safety standards, and ethical guidelines necessary to protect the millions of people already using it.
Until then, every health question posed to ChatGPT represents a small gamble: Will the response be helpful guidance or dangerous misinformation? Will it empower a patient to advocate for better care, or will it delay them from seeking treatment they urgently need?
For 40 million people today, that gamble is worth taking. Whether they’re right—or whether we’re witnessing a public health crisis in slow motion—remains to be seen.
